Application Helpful hints:
It is important that you fill out the application as completely as you can. (Please see our ‘helpful hints’ below.) This form is the only information we will have to work with as we begin our selection and writing process. Also, note that both you and your parent or guardian must sign the form, giving us permission to obtain information from the school and to publish the article about you.
Return your completed & signed form to the SHS Counseling office no later than
Friday, October 13th, 2017.
- You may fill out the attached paper application form by hand, in ink or download a digital copy.
- If you choose to complete the digital application; make sure you fill it out on a computer with an available printer. In the past, we’ve had students report that they lost their work when they found they could neither save nor print their completed application.
- Be sure to keep a copy of your completed application and/or the work papers you use to prepare it. You are likely to need the same information later in this school year for other forms. The time you invest in our application can make later applications easier to complete if you keep a copy.
You can either download the application as a PDF and complete the form manually or you can login to your Google account and make a copy of the document.
